The PWD identification card and the medicine and purchase booklets that let a person with disability claim the discounts and privileges under Republic Act 9442. You or your representative have to present the ID and booklet to get the discount, and this office issues all three. The same process covers a renewal and a replacement for a lost card.

    1. Reviews the application form and the submitted documents. If there are discrepancies the documents are returned to you; otherwise they go to the encoder.

    1. Checks the records and encodes the application in the DOH online Philippine Registry for Persons with Disabilities.

    3. If you are found to be registered in another municipality or city and that ID is still valid, the Processing Officer advises you what to do.

PDAO is published under the City Mayor's Office. Its charter names its staff in the process table itself, including job order staff.

Feedback and complaints

Answer the Client's Satisfaction Survey Form and put it in the folder or envelope provided. You can also text, email or call the office on 0923-726-5010 or 0917-777-9695.

Every 4:30pm the job order staff compile the forms and submit them to the Disability Affairs Officer II. Feedback needing an answer goes to the division or section concerned, which must reply within three days of receipt, and the office's answer is relayed to the client. Complaints by phone or online message are acknowledged, logged, checked for the identity of the complainant, analysed and acted on, and the client is told what was done by text, email or letter. Anything beyond the office's authority is forwarded to the head of office. Complaints written on the survey form are opened daily, passed to the Disability Affairs Officer II or Administrative Officer IV, investigated with the section concerned, written up in a report for HR, and the outcome given back to the client.
