Feedback and complaints

Fill in a feedback form at the office, or call the division contact directly. Each division publishes its own number alongside the City Health Officer's.

The office asks short questions, considers the client's experience, acts on what it hears and shares the feedback with the whole office. Complaints are filed on Form 3 from the Public Assistance and Complaint Desk, dropped in the PACD box, and answered by getting the facts, offering a solution and, where warranted, disciplining the employee concerned.
